Complement Activation during Hemodialysis with Cuprophan Dialyzer



Abstract
Various cardiopulmonar symptoms have ocured during the first 15 to 30 minutes of hemodialysis. The decreased leukocyte counts and arterial oxygen tension during the same phase has been reported, but its exact mechamism is under debate.
To investigate the relationship between the above phenomena and the activiation of complement, we performed serial measurements of arterial blood gas analysis, leukocyte counts and plasma complement during hemodialysis in the 6 patients with the stable chronic renal failure.
1) The plasma levels of C©ýa desArg were singificantly elevated at 15 minutes of hemodialysis and returened to normal within 4 hours. C©þa desArg did not change significantly during hemodialysis. The pleak plasma levels of C_(5)a desArg were noticed at 15 to 30 minutes and gradually returned to normal.
2) The major fall of total leukocyte counts occurred during the first 15 to 30 minutes, then gradually returned to normal within 4 hours.
3) Comparison of leukocyte counts with the corresponding C_(5)a desArg at each time had highly significant relationship (r=0.72).
4) The major fall of arterial oxygen tension occurred during the first 30 minutes and it gradually increased but persisted still lower level at the end of hemodialysis.
Thus we concluded that decreased leukocyte counts and arterial oxygen tension during the early phase of hemodialysis may be related with complement activation via alternative pathway.
